As an injured federal worker, you have the right to choose your own treating physician from the day you file, and you can change doctors later if the one you started with is not serving your claim. You Pick Your Doctor From Day One

When you are hurt on the job as a federal employee, you choose your own treating physician. Your agency does not assign you a doctor, and you are not required to use whatever clinic your supervisor points you toward. OWCP treats this initial choice as your right from the start of the claim [20 CFR 10.300(d)]. You can pick a doctor in private practice, a specialist, or a clinic that focuses on work injuries, as long as they are a qualified physician under the program.

This matters more than most people realize, because the doctor you pick at the beginning is not just treating your pain. They are building the medical record your entire claim rests on.

Why The Doctor You Choose Decides How Your Claim Goes

OWCP decides most of your claim on paper. A claims examiner who has never met you reads your file and looks for one thing above all: a clear medical opinion connecting your diagnosis to your federal job duties. In OWCP language this is called causal relationship, and it has to be supported by medical reasoning, not just a checkbox.

Here is where doctors who do not understand OWCP cost people their claims. A good family doctor might write “patient has lower back pain” and stop there. That note does nothing for your claim. What OWCP needs is a physician who explains how your specific work duties caused or worsened your specific condition, who fills out the federal forms correctly, and who responds when OWCP asks for more. When those reports are thin, claims stall, get denied, or drag out for months while you wait for treatment you should already have. When those reports are strong, decisions come faster and treatment gets authorized. If your current doctor treats your OWCP case like any other appointment, that is a real problem, and it is a valid reason to switch.

How to Change Doctors After You Have Already Started

Changing physicians after your initial choice is not automatic, but it is allowed. You submit a written request to OWCP explaining why you want the change [20 CFR 10.316]. OWCP approves the request when your reason is sound. Common reasons that get approved include moving to a new area, needing a specialist your current doctor cannot provide, or transitioning from general care to focused treatment for your injury.

There is one situation where you do not need OWCP’s approval at all. If your current treating physician refers you to a specialist, that referral does not require a separate request [20 CFR 10.316(a)]. The approval requirement applies when you decide on your own to change to a different physician.

Put your request in writing, keep a copy for your records, and be specific about why the change will help your care. A vague request is easy for OWCP to sit on. A clear one, such as needing a provider experienced with federal work injuries who can properly document your case, is much harder to deny.

The difference between a doctor who happens to accept your case and a provider who works with injured federal workers every day shows up in the paperwork. A provider who knows the system completes the Duty Status Report (Form CA-17) and the Attending Physician’s Report (Form CA-20) correctly, documents your work restrictions clearly, writes the rationalized reports that support causal relationship, and turns those documents around quickly when OWCP asks. They also understand what a work capacity evaluation (Form OWCP-5c) is and how it affects your benefits.

This is not about finding a doctor who tells you what you want to hear. It is about finding one who documents the truth of your injury in the way the federal system requires, so your claim is not held up by weak paperwork. If the provider you have now is not doing that, you are carrying a risk you do not have to carry.
